WP-Questionnaire Plugin
A while ago I hacked my WordPress installation to ask the user a stupid question before committing a comment. Today I noticed, there's already a plugin, that does exactly this (page is in German). It was written by Georg Bauer, aka Hugo. You can download the plugin here.
